Cancellation requests may be made this form (from the person who has registered for this conference). If request is received prior to September 1st, 2017, will receive a refund minus a $75.00 service charge. No cancellations will be accepted after this time.

- A signature form/sheet is presented to registrants at 10 minutes prior to the start of the event, and requires the registrant to sign in/out accordingly during the event.
- At the end of the event, you are required to sign out.
